I thought I'd try a quick experiment, using /dev/input/mice in .xserverrc. The cursor vanished as soon as I moved the stylus, so I'm guessing there is some kind of communication there.

Is it possible to use a Wacom with the default server, or do I need to  use Xfree or Xorg whenever I want the tablet?

So while it might work, you will need full X to control it better, ie to tell X how big it is and how fast you want it to react..
